BRIDGEWATER, N.J., April 1, 2016 /PRNewswire-USNewswire/ — Bridgeway Senior Healthcare, based in Somerset County, NJ, is partnering with Somerset-based University Orthopaedic Associates (UOA) to provide improved care coordination for individuals recovering from elective joint replacement surgery.

Enhanced communication among a patient’s orthopedist and primary care physician —both before admission to rehabilitation and post-discharge— is at the core of Bridgeway’s transitional care program. The goals of the partnership between Bridgeway and UOA are improved functional outcomes, care coordination and controlled cost after surgery for individuals recovering from elective joint replacement.
